Appropriate adaptive immunity is a critical determinant of malignant growth. The tumour microenvironment (TME) and host immune response are significant factors in tumour progression. Immune cell activation has emerged as a crucial determinant of tumour progression, while there is increasing recognition that the functional properties of TME cells can impact on immune escape in addition to proliferation and migration. Accordingly, certain TME cells with modulatory properties have been identified that contribute to immune evasion mechanisms. Cytokines released by tumour cells can affect the activity of various immune cells, by creating an immunosuppressive environment; this is particularly relevant in tumours with a high mutational load, where intratumoural heterogeneity is also a hallmark. In addition, the tumour microenvironment may induce lymphocyte exhaustion, a state of functional inefficiency acquired during the course of infection with chronic viruses. The high inter- and intratumoural heterogeneity of T cells and their inability to migrate to secondary lymphoid organs in most cancers may also be associated with T cell dysfunction. This article reviews the characteristics of TME cells and their functional consequences on the immune system in different tumour types. The contribution of TME cells to cancer immune escape is also discussed, with specific emphasis on the role of regulatory T cells, tumour-associated macrophages, MDSCs, and tumour-infiltrating lymphocytes as regulators of the tumour microenvironment.
